Some T-shirt design software programs design specifically for creating personalized iron-on transfers for T-shirts and similar projects. Many come with templates you can use to get you started.

Most importantly for your T-shirt projects, it supports printing to both transfer paper and printable vinyl on sheets of up to 13 inches x 19 inches (33cm x 48.26cm).
